  1. “Name of the Applicant” : Type your name
  2. “Date of Birth” : Enter your Date of Birth (DD/MM/YYYY) or select the Date of Birth
  3. “Address for Communication” : Type your complete Communication Address
  4. “E-mail ID” : Type your email address
  5. “Mobile No.” : Type your mobile number
  6. “Area of Interest” : Select the area of interest for which Internship is required
  7. “Educational Qualification” : Type your Educational Qualification
  8. “Studying at Present (Details)” : Type the studying at Present (Details)
  9. “Whether Presently Employed” : Select Yes/No
  10. “Experience” : Type Experience details
  11. “Extra curricular Activities” : Type the detailed Extra curricular Activities
  12. “Any other relevant information” : Attach/upload any other relevant information you wish to submit in PDF format (maximum 5MB size).
  13. “Upload Photograph” : Upload your Passport size photograph with Signature in the bottom in JPG format(maximum 1MB size).
  14. “Upload Documents” : Attach/upload the copy of Mark Sheet / Degree of Graduation / Post Graduation, a letter from their Supervisor / Head of the Department / Principal indicating their Status in Institution and No objection Certificate in PDF format (maximum 5MB size). Attached each file separately by clicking attach more.
  15. Click “Submit”.
  16. Click “here.” and print.
